Become an NDIS Participant
To start NDIS services, you need to become an NDIS participant by having your eligibility assessed and creating an NDIS plan with the NDIA for approved supports.

Contact us now on info@hfh.com.au for assistance to start your NDIS registration and find out if you’re eligible The NDIA will send you an Access Request Form that you will need to complete with your GP or specialist If you satisfy the eligibility requirements, you will receive notification of your Initial Planning Meeting, which can be held over the phone or face-to-face Once you have received your approved NDIS Plan via post or the NDIA Participant Portal, you can contact HFH care as an NDIS participant to discuss the supports in your plan
